I wouldn't worry about it. Usually, the reason will say "Bulk Administration" or something. It just means you took her survey and you can't take it again

 

i didn't get it, but many survey requesters will issue a qual to prevent you from retaking their survey, then revoking it when their survey is over and it's no longer needed. it's the worst way to have a mini panic attack, but most likely just cleaning house
