It is when I click on the textbox to input the completion code. Then, when I click continue on the error message, the little "a" icon in the top left for the page turns into a white box with a big red x in it.

 

@ChrisTurk It doesn't happen all the time. Only maybe 1 out of every 5 to 7 hits or so.
