I got a rejection for this same hit over a month ago. I emailed and then emailed again and finally got a response back saying that I didn't complete the survey. Which was not true. I did the whole entire thing. I asked them for proof and they never responded and never overturned the rejection. There was an issue with this particular HIT but they will not own up to it.

You can submit your worker ID as the code, and optionally message the requester to let them know that's what you did. Technically you could get a rejection for it, but it's super rare. I've done it hundreds of times with no issues.
