I messaged the requester and explained things. Hopefully they'll overturn it since I did another and got approved.

Recently I had a rejection because I didn't submit a code. I explained that I finished the survey and didn't get a code. But I honestly don't remember if I did put one in or not. I assume I did. But she let me retake it and reversed the rejection.
