Sometimes when that happens, you have to look and see if they added a new qual. Usually you would know this because all of your prior hits by them have been approved and there is an exclusion qual on the hit where your value says "none". Not all requestors do this (ari b lusting, I'm looking at you and the hit that was supposed to auto approve yesterday) but really should since so many are lazy and just edit a previous hit.
