Would you guys say that "Reaper, Grim pgs 14, 28-30 would have 2 mentions or 4? Those 28-30 mentions are messing me up, the examples given didn't show one with a mention spanning several pages.

Yeah, I know what you mean, whenever there are multiple pages like that, I'll subtract them an add 1 (useful when it's a large page range). So 30-28 = 2 +1 = 3, plus that single page = 4. So I'd put 4 for that example.
