4. In the Last Name input box, put your MTurk worker ID number.
5. Make up a number for the Company ID box.
6. Create your own password and confirm the password. Make sure it is something you can remember. You should try to complete these surveys in one sitting.
7. Demographic information is optional. You do not have to provide demographic information.
8. You should see that you have access to complete the HPI, the HDS, and the MVPI. You need to complete all three assessments.
9. You will not receive a payment code for these surveys. Please enter your MTurk ID below as the payment code.
