I contacted this guy and his advisor when I saw the problem in the HIT. The advisor was the only one to respond and told me they were working to fix it. I responded telling him that fixing the survey wouldn't help the dozens of people that already completed it without getting credit and asked if I should submit the HIT with my worker ID.

He ghosted me. They have also set the survey to inactive so you can't pull up their contact info. You can, however, find them at Perdue University Northwest (https://www.pnw.edu/). Their names are Hubert Izienicki (advisor) and Dino Topcagic (researcher).

I would 100% destroy the inbox of the IRB if I were you guys.
