-Survey formatting is terrible. ProTip: Zoom in using CNTRL+Scroll UP. At around 125%, the survey looked like it was on mobile and formatted itself properly. If surveys ever have that scrollable scale, try this trick, or press F12 and take the survey as mobile. It usually fixes itself one way or another.
