That error was from PC Queue Helper but disabling it didn't help.

In the F12 thing try going to Application at the top, then on the left sidebar "local storage" -> "worker.mturk" and count how many instances of keys there are that start with tv_

If there are more than 6, something is broken
If there are only 5, then it stopped tracking data (either you weren't submitting on worker or something else is busted)

 

A decent example for a nice exchange with a requester about a rejection. Bearnie Babies, take note.
